What you need to know

Registration
When you arrive, our receptionist will ask you to verify your mailing address, telephone contact information, and insurance information, along with requesting a photo I.D. This is necessary information to obtain so that we may contact you regarding your care and also to facilitate billing your insurance carrier.

Please be sure to bring your insurance card to each appointment and to present your insurance card, Medicare, Medical Assistance, or any other health care payer information to the receptionist at the time of the appointment.

Several insurance plans require a co-payment, which is due at the time of the visit. The receptionist will request the required payment from you at the time of your registration.

If the appointment is for a minor child (age 17 and younger), we do ask that the parent/guardian accompany the child to the appointment.

Medications
To assist in management of your medications please bring your prescription bottles and the names of any supplements, vitamins, or other non-prescription medication you are taking to each visit.

Prescription Refills
If you find the need to call for a refill, please check with your pharmacy first. They will obtain the necessary information from you and fax a medication request directly to your provider. Please note that it may take 48 hours to complete the refill process, so it is important to plan ahead, and it is best to discuss refill needs at your visit with your provider.

Emergencies
If you have an emergency, please call 9-1-1 or go to your local hospital emergency room.
