We are administering the Moderna vaccine. For ages 6 months and older.

If requesting a Booster dose, you are eligible 6 months after the completed series of Pfizer or Moderna, or 2 months after J&J.

It has been approved to mix and match vaccine types AFTER a completed series of one vaccine type.

COVID testing is being provided at all three of our locations Monday through Friday. Testing for symptomatic patients is done curbside from your vehicle. COVID testing requires a telephone visit with a provider and a scheduled time for testing, so call any of our clinics to schedule COVID testing. Same day testing is available Monday through Friday.


All Minnesotans over the age of 12 are now eligible to receive the COVID vaccine. Our allocation of vaccine from the state varies week to week, so you will find weekly updates here. By Friday morning each week our vaccine clinics for the upcoming week will be posted here. Vaccine availability will be first come first serve. Walk-ins will not be allowed.

We only administer the COVID vaccine at our Moose Lake clinic location. There is no charge for the vaccine, but your health insurance information is needed to bill for the administration of the vaccine, but you will have no out-of-pocket costs. Other local vaccination options are hospitals, pharmacies and county public health.
